Do not assess a producer solely on how quickly the first draft leaves the queue.
What is a useful defect-severity rubric?
Approve examples before the pilot:
| Proposed category | Illustrative issue | Proposed disposition |
|---|---|---|
| Critical control failure | Wrong-claim evidence, unauthorised disclosure, fabricated source or unauthorised decision | Stop release; investigate and escalate |
| Material production defect | Unsupported quantity, omitted documented scope, duplicate work or consequential calculation error | Return or correct with recorded technical review |
| Minor defect | Formatting or wording issue that does not alter meaning or usability | Record; correct according to the agreed acceptance rule |
| Evidence limitation | Missing or contradictory inspection information | Request evidence or escalate; do not invent a production answer |
Do not define materiality only as a percentage of estimate value. A small-dollar issue can still involve a serious authority or evidence problem.
What should the Inspection Rework Root-Cause Mapper record?
Use these initial origin categories:
- Inspection/input: evidence was missing or inconsistent at receipt.
- Production: adequate evidence existed but was omitted, misread or incorrectly entered.
- Carrier-rule ambiguity: requirements were conflicting, incomplete or changed.
- Integration: information was lost, duplicated or attached incorrectly.
- Review inconsistency: different reviewers applied different standards.
- New information: a genuine later development, not necessarily an earlier defect.
- Unknown/shared: attribution is not established.
For each return, record the provisional origin, confirmed origin, person resolving it, active minutes, elapsed delay and attributable cost. Allow disputed attribution to remain unresolved rather than forcing it into a supplier or inspector score.
How should the team run improvement meetings?
A proposed weekly review should examine the highest-cost recurring defects, not just the most frequent codes. Assign a corrective action, owner and retest date. A revised inspection requirement, training example or interface fix should be tested on subsequent files before being declared effective.
Exit gate: the operation can show whether rework originates in inspection, production, rules, review or integration—and what action follows.
